Xanthelasma

Xanthelasma is a condition characterized by soft, yellowish plaques or bumps that typically appear on or around the eyelids. These lesions are composed of cholesterol deposits and can occur in one or both eyes, often developing symmetrically.

While xanthelasma itself is not harmful or painful, it can be a source of cosmetic concern for many patients. The patches may gradually enlarge over time and, in some cases, cause the eyelids to appear puffy or aged.

Xanthelasma is primarily the result of lipid (fat) buildup, specifically cholesterol, in the skin. It tends to occur in adults between the ages of 35 and 60, and while anyone can develop it, certain risk factors increase susceptibility including high blood cholesterol or triglyceride levels, genetic predisposition, diabetes, obesity and poor diet.

It’s important to note that about 50% of people with xanthelasma have elevated blood lipid levels, making it more than just a cosmetic concern. It can serve as an external clue to internal cholesterol imbalance and a potential risk for cardiovascular disease.

There are several effective cosmetic treatments available for xanthelasma at Apex Skin Center. Laser Therapy

Multiple sessions may be required, and there’s a small risk of recurrence over time.

Surgical Excision

  • Suitability for larger plaques

Scarring is possible but often minimal, especially when performed by an experienced dermatologic surgeon.

Chemical Peels

Some superficial cases may respond to trichloroacetic acid (TCA) peels, which help dissolve the cholesterol deposits. Chemical peels are less invasive and are applied directly to the affected area. However, multiple treatments may be necessary, and this method is not suitable for deeper or more extensive xanthelasma.

Cryotherapy or Electrodesiccation Destruction

Cryotherapy involves freezing the lesions with liquid nitrogen and electrodesiccation uses controlled electrical energy to destroy the xanthelasma plaques. Both treatments can be appropriate for small xanthelasma areas with minimal downtime.

Recovery time after xanthelasma treatment is minimal. You may experience mild discomfort, swelling, or bruising which can be managed with over-the-counter pain medication and cold compresses.
